To assist our new patients with their first appointment. Prior to your appointment, please obtain copies of your medical records including any imaging. Please arrive 15 minutes prior to your scheduled appointment. While you will be pre-registered over the phone, it will still be necessary to complete paperwork at the time of your appointment, so please remember to bring your insurance card and picture I.D. You will be required to sign a HIPPA privacy act form at the time of your visit. If you have any questions regarding this form, please do not hesitate to ask.
It is recommended you write down any questions you may have for the physician in preparation for your visit. Please bring a list of all of your medications, including over-the-counter remedies, with you for your appointment.
If you have a HMO insurance, you are responsible for obtaining the necessary referral. If your referral is not available at the time of your visit, your appointment may need to be rescheduled.
If you have a co-payment, coinsurance or self-pay,, you will be responsible for payment at the completion of each visit. We accept visa, mastercard, discover, check, or cash. If you have further billing questions, please call our office and speak with the billing department at 321-728-0117.
